CoRDS Enrollment

If you or a family member have been diagnosed with a rare disease, you are invited to submit information to the registry.

We currently offer two process of enrollment via web or mail.

For web-based enrollment, click HERE to complete the web form or click on the icon on the left panel.

For mail-based enrollment:

  1. Complete the CoRDS Registry Form and click ‘submit’.
  2. CoRDS personnel will contact you and conduct a brief interview to gather basic information. CoRDS personnel will mail you the appropriate CoRDS consent form and questionnaire
  3. Read and sign the consent form, complete the 8 item questionnaire, and return the forms to CoRDS personnel using the pre-paid envelope provided.
  4. When CoRDS personnel receive the information, they will enter your information in the secure CoRDS database.
  5. CoRDS personnel will contact you annually to update your information. CoRDS personnel will also contact you regarding opportunities to participate in a clinical trial or research study.

*Participants are encouraged to contact CoRDS personnel at anytime during the enrollment process with questions.

Your information is stored in a secure database and is kept confidential.
